Collect evidence
- Order number and purchaser contact information.
- Product, model, and affected quantity.
- Carrier, tracking or PRO number, and delivery date.
- Delivery receipt showing damage or shortage notes.
- Photos of the full shipment, labels, packaging, and close and wide views of damage.
- A list of missing or damaged parts without attempting repairs.
Submit the report
- Keep all packaging and affected parts.
- Stop assembly or use if damage could affect safety or the claim record.
- Contact My Fitness Outlet and provide the evidence.
- Follow the written inspection, storage, repair, replacement, or carrier instructions.
Delivery damage concerns the condition in transit. A warranty claim concerns a covered defect or failure under the manufacturer’s terms. Do not file both processes for the same transit damage unless instructed.
